Handles the bulk loads sent by SqlBulkCopy.

A bulk load takes three requests: SqlBulkCopy first asks for the columns of the destination table, which the server answers with TdsBulkLoadHandler.GetTableColumns, then sends an insert bulk statement, and then the rows, which the server passes to TdsBulkLoadHandler.LoadRows. None of these requests reach the query handler.
